The goal
Fill this out every night for 14 days straight, then export and print it for the doctor's visit. Patterns over two weeks tell a story; memory doesn't.
Step 1 — Save to your home screen (in Safari)
Open this page in Safari. Tap the share icon (square with up-arrow) at the bottom of the screen → scroll down → "Add to Home Screen" → tap "Add." A new icon appears on your home screen.
Step 2 — Set a daily alarm for 8 PM
Open the Clock app. Tap Alarm → "+" → set time to 8:00 PM, toggle "Repeat" to Every Day, label it "Daily Log." When it goes off, tap the home screen icon you saved in Step 1.
Step 3 — Fill it out (60–90 seconds)
Be honest, not aspirational. Tap chips, drag sliders, type a quick note if anything was unusual. Skip fields you genuinely don't know — partial data is fine.
Step 4 — Glance at the Dashboard weekly
After ~5 entries, patterns will start showing. After 14 days you'll have enough to spot real trends.
Step 5 — Export & print before the appointment
On day 14, go to the Dashboard tab. Tap Export CSV (the raw data) and Print Doctor Summary (the formatted one-pager with your patterns). Email both to yourself and bring printouts.
If you miss a day: don't backfill from memory. Just resume the next day. A few gaps won't ruin the data — invented data will.
